"Yeah I think it was his earlier origins, there was some kind of lab accident that scarred his face, then it was either after the Fantastic Four got their powers or before that where he forged the mask either as a solo piece to cover his face and hide the scar, or was meant to be apart of the armor, but yeah it was right after he forged it and while it was still glowing hot did he put it on his face which naturally disfigured him further but I think in some instances that action made it fuse to his face. which is oddly similar to how the Blades of Chaos were 'fused' to Kratos' arms "

The irony being in most cases it was only a small scar. So he made things worse with the mask.

"That it was usually the case, something small, I think one was like just on the chin. But yeah he certainly acted like some models who to others see only a small minor blemish or maybe just a little tiny cut, but to them they see some horrible disfigurement. An example is from Batman The Animated Series, I forget the name of the episode but it involved a super model who suffered from that, if I recall all she had was some wrinkles the crows feet I believe they are called around the eyes."
